Studies show that between 40%-75% of older adults do not take their medications properly.  Older adults are more sensitive to drugs because of their slower metabolisms and less effective organ function.  Older adults also have more trouble remembering what medications they have or have not taken.

A substantial risk for older adults is an adverse drug interaction of two or more drugs.  Below is a sample list of Medication Treatment Risk Factors:

  • Taking 5 or more medications
  • Inability to manage medication-related devices
  • Taking 12 or more doses per day
  • Prescriptions from more than one physician

  • Significant changes in medication treatment regimen in last 3 months
  • Difficulty caused by language barriers, dexterity problems, poor eyesight or cognitive impairment

  • Medications requiring therapeutic monitoring
  • Recent discharge from a facility
  • Sub-optimal response to drug treatments
  • Suspected non-compliance

Astera Care offers a comprehensive monitoring system designed to mitigate the dangers posed by drug-related errors.  Our Medication Management program ensures drugs are taken properly, on a set schedule and in coordination with the prescribing physicians' orders.
