The In-Home Assesment

 The first step for all new clients is an In-Home Assessment.  An Astera Care Care Manager schedules a time to meet with the client (and the client's family, if necessary) in the client's home.  The Care Manager reviews the client's medical history and performs a detailed evaluation of the client's physical, mental and psychosocial status.
 

The In-Home Assessment provides the Care Manager with the information required to draft a Care Plan:  a clearly documented evaluation of the client’s current status and an outline for the most appropriate course of action for the client's care.

The In-Home Assessment is extremely valuable in determining, among other items, the following concerns:

  • Can the client continue to safely live alone?
  • What are the client's current and potential needs?
  • Are the client's medications being properly managed and taken per prescribing physicians' orders?
  • Do any changes to the home need to be made in order to render it safe for an older adult?
